The Improving Limited English Proficiency Patients’ Management of Diabetes program combines a community health worker/ navigator model with our cultural mediator approach to support Spanish and Somali-speaking patients in managing their diabetes. The program’s bilingual navigator/ educator/ coach for each language group will work with the patient and their health care team to ensure that each patient’s treatment plan is culturally tailored and grounded within the individual’s context.
