Public Records Requests
If you would like to request public records from PHPDA, you can either:
- Email us at publicrecordsrequests@phpda.org with your request.
- Fill out this request form and submit the completed form by email, by handing it to the current Public Records Officer in-person at our office, or by mail to our mailing address (1200 12th AVE S, Quarters 2, Seattle, WA 98144).
While the request form is available for use, it is not required to submit a request.
Our Governing Council, Finance Committee, and Program Committee pages contain publicly available records such as meeting agendas and minutes.

If you have questions about our grant programs, please email our grant staff at grants@phpda.org.
General Inquiries
If you have general inquiries about PHPDA unrelated to public records or grants, please reach out to us via email at officeadmin@phpda.org.
