REACH provides intensive case management to immigrants and refugees that are homeless with substance use disorder and behavioral health conditions. There is a need to provide these clients with mobility and medical equipment to help clients deepen and sustain positive health outcomes. REACH does not currently utilize this equipment due to lack of financial resources.
The proposed project will enhance the existing intensive case management services REACH case managers provide in partnership with nurses from NeighborCare Health. The first step will be to identify all medical and mobility equipment needs among current clients. To do so, REACH case managers and NeighborCare Health nurses will review results from clients’ initial medical assessment and develop a list of needed equipment. This will occur once for existing clients and on an ongoing basis for new clients within their first three weeks of working with a REACH case manager. The REACH health coordinator will then receive a list of equipment needs, which could include glucometers, lancets, testing strips, blood pressure readers, oxygen tanks, breathing machines, nebulizers, and electric scooters. A pre-determined quantity of each will be purchased at the beginning of the project launch to receive bulk pricing and reduce turnaround time for new clients to receive needed equipment. The REACH health coordinator will schedule distribution and education site/home visits for REACH case managers and NeighborCare nurses to deliver the equipment and provide instructions for how to safely use the equipment. Ongoing, check-in visits will occur by case managers as part of their case management services.

They advance their mission through a four-pronged approach: medication-assisted treatment, outreach, research, and training. ETS uses a comprehensive approach to treatment which combines medication assisted treatment with wraparound services such as medical monitoring, counseling, and drug screens. They also support the REACH program which provides street-based, case management and outreach services to adults living outside in the greater Seattle area.
